A 64-year-old male patient was admitted to our clinic having been diagnosed with an ischaemic cerebellar stroke. The symptoms began a day before, with ataxia, speech disturbances, and apraxia of the right body side. The laboratory tests at admission showed no abnormalities. Computed tomography showed a distinct, compact structure in the lumen of the ascending aorta and aortic arch. All the branches of the aortic arch were intact, but there was an acute closure of the left vertebral artery. To prevent further embolisation, we performed an emergency surgery. After median sternotomy, the ascending aorta was cannulated at the level of sinotubular junction. Then we cannulated the right atrium with a two-stage cannula, and a left ventricular vent was placed. The brachiocephalic artery and the left carotid artery were identified. The patient was cooled down to 30°C and 1 L crystalloid cardioplegia was given. The aorta was cross-clamped. Within 5 min of circulatory arrest the aorta was opened to inspect the arch. A large intraluminal thrombus was attached to the aortic wall with a pedicle, going along the aortic arch and partially occluding the left subclavian artery. After removal of the fragile and partially fragmented thrombus, the aortic arch and its branches were inspected. The aortotomy was closed. After rewarming and reperfusion, we could wean the patient from the cardiopulmonary bypass. The first day after surgery, our patient was awake and extubated. The symptoms of stroke were still present, es- pecially coordination disturbances and indistinct speech. In postoperative course, we included oral anticoagulation with phenprocoumon into his therapy, and on the fifth postoperative day, we referred him to a neurological clinic for blood coagulation diagnostics and intensive rehabilitation. #cardiacsurgery#cardiology#teamweymann#stroke#thrombus

This type of blood clot, also called an aortic #thromboembolism , tends to become stuck right where the iliac arteries, which supply blood to each hind limb, split off the aorta. This clot cuts off blood supply to the back legs, resulting in weakness or paralysis of those limbs, excruciating pain (your poor kitty will be screaming in agony), and toes that often become cold to the touch. A saddle thrombus tends to occur without warning, but what causes it? The vast majority of cats that develop these blood clots have underlying heart disease, most commonly hypertrophic cardiomyopathy (HCM). Many of these kitties never show prior signs of cardiac illness, so it’s extremely important that you take your feline companion to the vet for routine health checks, including a physical exam to detect heart murmurs. Though some cases may be medically treated, many of these cases sadly end in euthanasia due to poor response to proper pain control and blood clot dissolution. Fortunately, however, the development of a saddle thrombus is relatively uncommon, even amongst felines with heart disease. And cats aren’t the only ones at risk for developing hind limb clots – dogs with certain medical disorders may be at risk too. Remember that thrombi found in areas of rapid blood flow (like in the heart or aorta) have very distinctive alternating layers of red and white, called “lines of Zahn”. The red layers are red blood cells, and the white layers are fibrin mixed with platelets.

Blütverdünner, Schmerzen und sexy Strümpfe. Natürlich muss dies nicht zwangsläufig bei jeder Frau, die die Pille nimmt, eintreten. ABER es kann! Und wenn ihr dann noch raucht, dann habt ihr das große Los gezogen. Erkundigt euch! Nehmt nicht alles hin, was der Arzt euch sagt! Hinterfragt und seid kritisch! Immer! Und ein kleiner Rat zum Schluß: Pillen der ersten Generation sollen weniger Nebenwirkungen haben, als welche der 2. oder 3. Generation, was natürlich aber auch nicht immer gegeben ist. This is a resected uterus showing a molar pregnancy, or in more medically appropriate terms a gestational trophoblastic neoplasia. It's an abnormality of the placenta and happens when tissue that normally becomes a fetus instead becomes an abnormal growth inside the uterus. Even though it isn't an embryo, this growth triggers symptoms of pregnancy.
It is the result of a genetic error during the fertilization process, with 2 main types - A benign or malignant types.
Benign includes complete and incomplete pregnancies.
In the complete form, a single sperm fertilizes an empty ovum so there's no fetal tissue present, while the incomplete form contains fetal tissue due to a normal ovum being fertilized by two sperms.
The malignant form gives real troubles, it may progress into invasive moles and in some patients lead to choriocarcinoma with pulmonary and CNS metastasis and trophoblastic pulmonary emboli.
Patients typically present with uterine bleeding and hyperemesis gravidum characterized by severe nausea, vomiting, weight loss, and electrolyte disturbance.
