How often do you and your colleagues complain about electronic health records (EHR)? At the same time, we take their utility for granted!
Downtime requisition forms? Calling the lab for results? Manually charting vitals in the OR when trying to perform massive transfusion? Documenting procedure notes by hand? And don't get me started on writing orders/notes on paper charts! These are just some of the tasks simplified by EHRs. 🖥📂
On that note, just because the EHR goes offline does NOT mean patient care should suffer. The hospital functions 24/7, 365, and with/without an EHR. It's unacceptable for people to use EHR downtime as an excuse to provide suboptimal care, so be familiar with alternatives! 👨🏽⚕️💉

"We need to keep vaccination rates up to maintain herd immunity. If we do not, deadly diseases like Measles will make a come back and kill millions of Americans."
According to my calculations, we should all be dead from measles, and here is why:
Protective herd immunity is a theoretical principal that applies when a population reaches a critical point of vaccination known as the herd immunity threshold. For measles, this ranges from 83 to 94% of a population.
Let's use an average life expectancy of 80, and actually estimate current vaccine coverage rates. There are two primary groups to factor in.
1. Those considered not immune to measles. Current vaccine policy assumes a person becomes immune once they have received two does of MMR. Therefore, anyone receiving one or zero doses, we will consider not immune.
2. Those considered immune to measles after receiving two does of MMR.
In 1989 the second MMR was recommended, and should be given by 12 years of age. So to simplify our estimate, let's say everyone 12 years and older in 1989 did not receive two doses, and is considered not immune (in 2018 this is everyone 41 or older). This age range accounts for approximately 47% of the American population (who are not immune to measles).
If only 53% of Americans are fully vaccinated against measles, this leaves a minimum coverage gap of 30% between actual coverage rates and the herd immunity threshold. This does not even factor in confounding variables such as vaccine refusal after 1989, and vaccine failures (primary or secondary).
So according to my calculations, we should be seeing massive and widespread measles outbreaks in America today. My estimates show that herd immunity currently cannot exist, and is therefore not providing a protective mechanism against measles across the general population as a whole. Due to this large coverage gap, vaccination coverage rates become irrelevant. They are only mentioned by those who promote vaccines as an excuse to vaccinate everything with a heartbeat. #generationsagainstvaccines
