Flight Medicine Friday! One of the fantastic aspects about my residency program is the ability to transport patients via helicopter. I think it adds a great deal to my Emergency medicine education and I wanted to highlight my favorite parts! . 1. You are by yourself. This is probably the most challenging part of flying in a helicopter, it is just you, the nurse and the pilot. You are able to call an attending for help, but when stuff goes down, it is on you to make a decision about what to do next. It not only pushes you to new levels but also exposes some of the gaps in your knowledge that will make you better for next time. . 2. The patients are sick. If the patient was healthy and stable, a ground ambulance would take them to the hospital, this is not the case with helicopter patients. They need to be in the hospital NOW. The patients can be medically complex or even unstable. You always have to be on your toes when transporting this group of patients whether it be a pediatric trauma or a heart failure patient on ECMO. . 3. Hospital in a box. Have you tried intubating a 350lb patient in a 5ft x 6ft x 7ft space? Have you had your IV lines freezing from the subzero temperatures? Has your patient vomited in their endotracheal tube from motion sickness? These are obstacles you would not regularly encounter in the hospital. So what do you do? The helicopter and almost everything you need to troubleshoot difficult situations. It is up to you to MacGyver some sort of solution to get your patient to their destination. Again, challenges that make you a better doctor. . Lastly, I am not going to sit here and tell you flight medicine is not dangerous. You are thousands of feet in the air and a whole host of things can happen. Flight medicine isn’t for everyone. If something were to happen to me, I’ve come to terms with that possibility.
